About the role
Our client provides a range of specialist community palliative care for patients with life limiting illness. Provide clinical expertise and encourage a high quality of life for those living with life limiting illnesses and their whanau. They are a dynamic team who are constantly striving to improve how they operate and everyone who works there plays a crucial part. Offering 0.7 FTE to 0.8 FTE.

What you have to do Deliver exceptional support and services to patients and whanau in community setting
Provide holistic assessments and care planning programmes
As part of a team collaborate in partnership with the patient and whanau to provide wrap-around care
Ensure our patients are able to live their last years of life to the highest of standards
Skills & Experience A current New Zealand Practicing Certificate as a Registered Nurse
Experience in completing full, comprehensive assessments
Palliative , oncology, haematology or aged care experience
Be experienced in wound care, medication and pain management
Exceptional communication skills to effectively liaise with other health professionals and multi-disciplinary teams
Multi-cultural awareness
Right to work in New Zealand
Full clean driving licence to work in the community
