About the Role
Embark on an exciting journey with a brand new team as they launch a groundbreaking residential Dual Disability service in Waitemata! As a seasoned Registered Practitioner, you'll play a pivotal role in guiding Whai Ora through their transition, providing 24/7 support in our Swanson service.

What You Have to Do Apply your expertise to integrate clinical and rehabilitation knowledge for exceptional client services
Manage medication plans, coordinate GP visits, and support Psychiatric appointments
Conduct SNAP meetings every three months, diligently updating client goals
Engage in one-to-one assessments, reviews, facilitation sessions, and monthly mentoring
Skills and Experience Demonstrate clinical training and substantial experience in dual disability support
Exhibit a willingness to embrace ongoing professional development
Showcase flexibility, robust people skills, attention to detail, and organizational prowess
Maintain current registration with a professional body under the HPCA 2003 Act
Possess a minimum of two years of post-qualifying experience in the mental health sector
Ideally, bring experience in a residential setting
Hold a valid New Zealand driver's license
