CARE MANAGER Navigate Is a Regional Intellectual Disability Supported Accommodation Service (RIDSAS). We work with people with an intellectual disability who have complex behavioural needs and/or who have offended.
The Care Manager is legally entrusted with the compulsory care and rehabilitation of individual care recipients. They are responsible for developing and implementing an individual's care and rehabilitation plan and for ensuring regular clinical reviews are undertaken.
Our RIDSAS services focus on the provision of intensive rehabilitative support to enable people we support to develop the skills they require to participate in and re-join their communities. This role will work in partnership with a multi-disciplinary team of disability/health practitioners and other related professionals.
