They are responsible for ensuring referral information provides our teams with enough information to plan the right level of support for people in their homes.We are seeking an experienced Registered or Enrolled Nurse to join our newly established intake team in Albany. This is an exciting opportunity that will see you use your clinical knowledge and experience to help establish a brand-new service helping our local branch teams meet the needs of our clients in their homes. You will work closely with our Referrals Coordinators and local service delivery teams to ensure the services and care we provide to our clients meet their individual needs while also understanding the variety of Home and Community Support Contracts we provide across New Zealand.The hours will be Monday to Friday - 8am to 6pm on a rostered basis but are very flexible to suit your lifestyle. Full time, part time and job share opportunities will be considered.A few of your responsibilities:Screen incoming referrals to identify specific health needs and ensuring these meet criteria for a range of service provided by Healthcare NZEffectively liaise with referrers to ensure referral information is accurate and matches service delivery requirementsUnderstand the requirements of our funders and the contracts we hold with themEnsure the accurate and timely set up, initial contact, and on-boarding processes for new ClientsEnsure complete and accurate data entry into the Client Management System and other systemsManage and build strong relationships with Branches, nurses and kaiawhina/support workers to meet the changing needs of the clientIdentify opportunities to develop clinical innovation that is sector leading and promotes best practiceYou will have:Five years post registration experience as a Registered or Enrolled Nurse and a current annual practicing certificateExperience and knowledge of the health/disability or community sectorStrong critical thinking and problem-solving skills, the ability to manage the unexpected or difficultStrong relationship management capabilities at all levelsA high level of culture sensitivity with experience in relating to people from different cultural/ethnic backgroundsExcellent interpersonal skills and communication skillsPassion and expertise to working collaboratively in a team environmentCurrent and relevant skills and experience in clinical decision makingPlanning, organisation and time management skillsMust be competent in the use of computer applications and databasesAbility to work under pressureExperience as a Support Worker or in care coordination is advantageousUnderstanding of interRAI is an advantage but not essentialWhy join HealthCare NZ:Be part of an exciting new team, developing a brand-new serviceComplete unique role with an abundance of diversityPart of a national Healthcare companyThe ability to make a positive impact in the lives of othersFlexible working hours to fit around your lifestylePotential to job share can be consideredMena he tangata ngakaunui ana koe ki te tautoko i nga momo tangata, tena pea, kei konei he turanga mou.
