Transition Coordinator - Forensic Mental Health Services A exciting role to support the transition of tangata whaiora from the Forensic Service to the Community. Fixed Term full-time position for 1 year Mote tunga | About the role Forensic mentalhealthservices specialise in the assessment, treatment and risk management of people with a mental disorder who are currently undergoing, or have previously undergone, legal or court proceedings.
This position is a clinical position allocated specifically to identify and then work with tangata whaiora and staff to transition inpatient tangata whaiora through to community placements by targeting barriers. You will be working closely with tangata whaiora and their whanau, NGO partners and Forensic Inpatient and Community MDTs.
Ko wai koe | About You Establishing self as a key member of the MDTto support with identifying the specific needs of each of the tangata whaiora whocurrently have no discharge pathway.Working with planning and funding andcommunity providers to identify where needs can be met including looking atalternate care pathways.Development of comprehensive transition planswhich include the consumer, MDT members, whanau and support agenciesAssisting with the transition process-supporting the tangata whaiora, their wahnau, the MDT and the community providerAbility to engage and work with clinical MDTteams to identify and address individual tangata whaiora needs and to identityappropriate discharge pathways including any barriers to discharge. Ability to develop and maintain therapeuticrelationships with tangata whaiora and their whanau and work in partnershipwith them to ensure successful and appropriate discharge pathways.Ability to work collaboratively with variousstakeholders including clinical inpatient / outpatient teams and ROSC.Comprehensive knowledge of communityproviders, and the ability to develop and maintain strong working relationshipswith community agencies and providersAbility to problem solve and think creativelyWill be resilient and adaptive - showscomposure, resolve and a sense of perspective when the going gets tough.
