The Role Asa therapist at Connect2Care, you will be joining amultidisciplinary team. You will provide evidence-basedservices to people through multiple service deliverymodels, including telehealth, community visits, ¢re-based services. Referrals may come through theNDIS, Medicare or privately. It is a mobile role, with theoption to work from home or our hubs when not seeing clients.The role will involve the following:

Assessing and providinginterventions for children & adults livingwith a range of disabilities
Working within the NDISto maximise clients' level of function, capacity& social engagement
Assisting clients withassistive technology assessment, recommendations andapplications (in relevantfields)
Timely documentation& report writing
Managing a variedcaseload
Meeting companyprofessional expectations (e.g. meeting billables &effective client management)
Building relationshipswith key stakeholders, including therapists,clients, & referral sources
Working across leading softwareplatforms & online tools

Requirements To apply for therole, you will need to have or be able to obtain thefollowing:

Relevant Tertiaryqualifications in related field
Discipline-specificprofessional registration (e.g.AHPRA/SPA)
Working withChildren Check (WWCC or Blue Card)
NDIS WorkerScreening Check
Police Check& International Police Check (onlyapplicable to individuals who have livedoverseas)
Driverslicence & access to a reliablevehicle
Carinsurance
ProfessionalIndemnity & Public Liabilityinsurance
