This involves identifying and resolving issues as and when they arise to maximise rehabilitation potential. This short term service aims to return clients back to their baseline level of function and if that is not possible, to plan for long term support.This role is about working with our already established team of Nurses, Coordinators, Support Workers and other Allied Health within the community setting.
