Registered Mental Health Professional-Case Manager Are you ready for a new challenge? An opportunity has arisen for a Registered Health Professional with Mental Health experience to join the Adult Community Mental Health team on a permanent basis at Tauranga Hospital. This is an excellent opportunity to work within a highly supportive and friendly team. The successful applicants will be joining a multi-disciplinary team with a focus on the Recovery Model for individuals who have a serious mental disorder. The aim of this team is to work with referrers, health partners, families/whanau and the community to provide effective triage, assessment and management of Tangata Whaiora who are requiring our services. We work alongside the Tangata Whaiora with the core values of patient centred care, and we achieve this with a focus on maximising health while promoting independence in the community. You will be provided with support from the Clinical Lead around caseloads and clinical issues, and there will be opportunities to develop your skills within the team – including progression via PDRP or CASP. The nature of the work itself means that there is a lot of variety, so it is different and diverse each day. We have permanent positions with both part- and full-time hours available. Interested? To be successful in this role you will have: Registration with a current practising certificate At least two years of experience in a mental health setting A passion and a drive to make a real difference through your work and have a strong cultural awareness.
