Nurse Practitioner, Community Health Service at Te Whatu Ora - Health New Zealand Counties Manukau, Auckland Nurse Practitioner–1.0 FTE fixed term position ( full time / part time) Would you like to contribute to the future of health care in the Counties Manukau area? We are looking for a Nurse Practitioner to provide clinical expertise and leadership to the Hospital in Home and Northern Community Teams within our Specialist Community Health Service. This is a fixed term position for 13 months to cover parental leave.
We are interested to hear from any Nurse Practitioners who might be interested in full or part time hours.
In this position you will work at the top of your scope supporting the community nursing teams within a multi-disciplinary approach. This will include doing joint home visits with nursing and allied staff to reduced hospital readmissions and developing multiple clinical pathways. You will work closely with our senior medical officers to deliver acute care to patients within their own homes as well as virtually including remote patient monitoring with wearable devices.
It is an exciting time to join our Nurse Practitioner team of four where you will be well supported with mentoring and supervision from our senior medical officers to further develop your skills and expertise.
This position is working 8-4:30pm in a 7 day a week roster. There is an on- call roster that this role contributes to.
• Registered with the NZ Nursing Council
• Current APC with the scope to work as a Nurse Practitioner
• A minimum of 5 years recent clinical experience in a relevant area
• Completed a clinical Master's degree
