Would you like to be part of a Healthy Ageing Service - Te Ara Tiaki Pakeke, committed to supporting older people to Get Well, Stay Well and Live Well in supportive communities? Are you a registered health professional with at least three years' experience with a current annual practising certificate? We are looking for a person to join our service to undertake Needs Assessment and Service Coordination (NASC) . The key objective of NASC is to maximise and support the ability of older people (usually over 65 years, though can be younger) to identify their needs and goals and make decisions on how these both can be met. NASC assess needs, identify support options and access support services in support of a client. NASC work with clients and their whanau (family), together with other stakeholders to build integrated supportive plans that focus on maximising a person's independence so that they can participate as fully as possible in society, in accordance with their goals, culture, resources, and abilities. This role may be fulfilled by Registered Nurses, Occupational Therapists, Physiotherapists and Social workers.
