Community Development (Community Services & Development) Te Manu Toroa has two GP clinics: Tauranga Moana City Clinic and Te Akau Hauora with an enrolled patient and client population of over 10,000. Other services include Hei Oranga (Primary MH & Addictions, IMPHA), Dental Services, Mental Health & Addictions Services, Pepi Ora, Whanau Ora, Whare Oranga, Community Dietician Service, Specialist Cardiac Service, Diabetes and Respiratory Services, Kaupapa Maori Advanced Nursing Service (KMANS), Hapainga (Smoking Cessation), Kahurangi/Social Support Services. Te Manu Toroa is a member of Nga Mataapuna Oranga PHO which is a Whanau Ora To consolidate a Bay of Plenty Regional Mental Health NASC model of service delivery to Maori (as developed) that fosters tangata whaiora and whanau empowerment.  To manage a caseload of up to 15 tangata whaiora utilising effective NASC assessments and coordination of care and using professional knowledge and skills in accordance with relevant professional scope of practice  Undertake needs assessments to identify individual strengths and supports and activities required that are likely to lead to resilience and outcomes for the tangata whaiora and their whanau Facilitates the access of adults, to a range of community-based resilience, recovery and support-focused services. Access to those services will be facilitated through the processes of comprehensive support, needs assessment and prioritisation of access according to identified needs. Key attributes are: Health or Professional Tertiary Qualification Registered to a professional body with current annual practicing certificate. Minimum two years recent experience working in mental health and co existing problems. Understanding of Te Tiriti o Waitangi Experience in a Kaupapa Maori environment an advantage.
