We are committed to delivering a high-quality Kaupapa Maori service that supports individuals and whanau (family) to achieve their hauora (health) and social well-being aspirations.About the Role - Mo te tungaIf empowering individuals and whanau is important to you, join our Hauora (Health) team and be a part of the Comprehensive Primary and Community Care (CPCT) initiative being rolled out in our community. Our Kaiawhina (Advocate) will work closely with individuals and their Whanau (family) to support them to access primary care and secondary services in a timely manner. Key to the role is understanding the health system and having the ability and confidence to guide individuals and whanau to essential services they require. This role would suit candidates who have excellent knowledge of the health sector, and or community. Relationship building and communication skills are essential as you will be providing practical advocacy and support to whanau by. Supporting whanau to engage in activities to promote health and well-being programs such as screening and immunisation. Working alongside whanau and Key partners to identify and overcome barriers to accessing health care.Support Maori-led and Pacific-led responses, including tangata whenua- and mana whenua-led care coordination to deliver mana motuhake (autonomy) and Maori self-determination. Co-creating, pro equity health planning and coordinating care for individuals, whanau and communities.Demonstrating commitment and understanding of simplify and intensifying care based on risk profile and risk of health inequities.Professionally taking a stand for equity and commitment to helping all people achieve equitable health outcomes.Establishing good working relationships with key partners.You should be comfortable with and or have an understanding of clinical and non-clinical services and Te Ao Maori models of practice.
