This includes:Empowering patients to take control of their health and wellbeingEncouraging good management of various health issues that patients want addressedDiscuss how to be more active by doing things they enjoyAs you will also have the flexibility of working in the community, you will have a significant impact on people's well-being and ability to make progress. The key word is "support", walk alongside people, supporting them to achieve their goals.
