Established in 1995, our client provides quality health, social and education services for all who reside within the rohe of Tuwharetoa Ki Kawerau and Eastern Bay of Plenty.He aha te tunga | About the roleManaging approximately 10 - 15 caseloads per quarter, the key purpose of the Elderly Support Practitioner role is to ensure that support services are being seamlessly delivered to Uri Haumate (uri) of Tuwharetoa ki Kawerau Hauora. The successful candidate will ensure that older people experiencing (or perceived to be experiencing) abuse and neglect have timely access to appropriate local services that respond to ensure their immediate safety, and support them to have greater control over their lives.Key responsibilities of the role include:Successfully implement interventions and strategies that reduce the risk of social isolation, neglect and/or abuseBuilding rapport and working collaboratively with uri and their whanau including caregivers to develop goals and strategies to keep uri wellIdentifying and monitoring any health and wellbeing issuesPreparing referrals to ensure kaimahi respond to immediate crisis as soon as possibleEnsuring the client is fully informed and involved in decisionsWorking collaboratively with whanau and caregivers to empower and support uriIdentifying needs and preparing referrals to external agencies for additional servicesProviding education and health promotion to strengthen uri well-being and resilienceCompleting reporting requirements, ensuring strict client confidentialityFile management, capture date and update progress detailsEnsuring all documentation is accurate and are managed and filed as per protocolNga pukenga me whai | The ideal candidate must:Be a Registered practitioner/social worker with a current practicing certificateA minimum of three years' New Zealand experience, in a similar roleProven experience working in a kaupapa Maori environmentKnowledge of TKKH and the Eastern Bay of Plenty CommunitiesStrong relationship and network managementExcellent organisational skills, with the ability to meet deadlinesExcellent computer literacy skills, with proficiency in all Microsoft Office functionsBe positive, adaptable to change and resilientKnowledge of Tuwharetoa ki Kawerau tikanga and kawa is desirableCompetency in Te Reo Maori and a good understanding of tikanga Maori is desirableIt goes without saying that you will be professional, trustworthy, confidential and have excellent attention to detail.
