About the Role The Community Participation Facilitator is responsible for supporting facilitated pathways for people to access mainstream community opportunities. The Community Participation Facilitator will work collaboratively with the Community Participation Manager with the focus of developing community connections for people based on their interests, skills and goals that provide a mainstream pathway out of Hohepa provided support and into community supports or employment.
Responsibilities: Coach people we support to prepare for opportunities within the community e.g. job interview. Document changes to support and daily routines in Iplanit. Maintain spreadsheet of community connections Identify interests and skills of people and facilitate community connections that can build into ongoing community supports. Accompanying people in initial steps and stages of building community connections e.g. meetings and interviews. Maintaining key relationships with internal and external people and organisations. This is a 40 hour/ week position. Whilst the hours for this role are predominantly between 8:30am -5pm Monday to Friday, flexibility is required to ensure people are achieving their good lives across the week. This will include time in the evenings and at weekends as the need arises.
