About the Job
Reporting to the Clinical Manager, you will provide comprehensive, wrap-around care to our clients, who require the highest level of rest home care, all within the comfort of their own homes.You will also:
Conduct comprehensive assessments (InterRai) and 3-monthly reviews.Monitor client health and coordinate care plans.Accompany clients to appointments, ensuring a holistic approach to their health and wellbeing.Utilise multiple systems including ECA, Clinical Portal, interRAI and the organisations proprietary system.This role is 0.8 FTE (32 hours per week) and is perfect for someone who can work efficiently within time constraints and is committed to improving the lives of individuals who are unable to meet their own health care needs.
The ideal candidate profile: Registered Nurse with current practicing certificate.Previous experience in community health, aged care or a similar setting.Technology savvy with the ability to navigate and utilise multiple systems.Exceptional time management and organisational skills.A compassionate approach and a commitment to providing patient-centered care.Ability to work independently and as part of a team.
