NZ RegisteredHealth professional with a Mental Health Scope of Practice & current Annual PracticingCertificate.  2 years experience within specialistmental health My strength, is not the strength of one, it is the strengthof many Mo te tunga | About the role Undertakes all components of case management, including:comprehensive assessment, risk assessment and management, recovery, andtreatment planningProvides client-centred care, utilising evidence-based treatmentstrategies including, where appropriate, talking therapiesTake part in the local crisis response service roster.Monday to FridayWe are looking for a Case Manager with a Nursing or Allied Health background, who has the clinical skillsrequired to assist people with current or ongoing mental health problems and tomaintain their place in the community. Thisrole will be working within the community and within our integrated model ofcare.
