Your primary function will be to support whanau witheducational resources, advice, a care management plan and facilitating accessto the relevant support services ensuring the best pathway of care isestablished for each child. This is a permanent part-time positionworking 48 hours per fortnight (0.6FTE) located in Nelson or Wairau withdistrict-wide responsibility. You will need be able to workautonomously with effective time management and organisational skills.You will have a relevant tertiaryqualification such as a degree in an Allied Health discipline or level 4certificate or higher in Health and Wellbeing.
