This service has been established to work in collaboration with health stakeholders and the wider community to enable older people to receive care in community centred around their needs. The service is committed to maintaining the best possible health and the greatest level of independence for older people, with avoidance of unnecessary hospital stays a key objective. Te Ara Tiaki Pakeke promotes equity and sensitivity to different cultures, and is responsive to the voice of pakeke and whanau. Mou - About youIn the role the successful applicant will deliver Home and Community Support Services to clients who have been recently discharged from hospital or who have experienced a significant change in their circumstances in community, including requiring palliative care. This support may include personal cares (hygiene cares) and to a lesser extent home help (housework), together with supervision and support with rehabilitative programmes, with the focus on re-enablement and recovery. The work will be rostered across a seven day week, 5 consecutive days, with either a 7.30am-4.30pm, or 9.30am-6.00pm roster.The successful candidate will be passionate about enabling the best possible health, and highest standards of care for older people.
