Acute Demand is a Canterbury wide network of services designed to provide community based care for patients at risk of presentation to the Emergency Department or Hospital admission.
About the Role
The Acute Demand Coordination team is based at Pegasus House and is responsible for providing ongoing support in the provision of services for the Acute Demand team, within both the primary and secondary care environments.
You will provide clinical advice, leadership, support and mentoring to the Coordination Administrators and their Team Leader. You will work collaboratively with general practice teams, hospital, ambulance and community services and provide advice to referrers. In this role, you will help to improve and develop service systems and processes to ensure that our patients and communities are provided with an exceptional level of care.
Skills and Experience
Registered Nurse with at least 5 years’ experience and have a current Practicing Certificate.
A proven track record of service excellence and the ability to develop positive collaborative relationships within the health environment.
Must be an excellent communicator, with an approachable and friendly manner
Display a hardworking attitude, high levels of initiative, plus a thorough and conscientious approach.
Great organisational skills and attention to detail, as well as good computer skills.
