We have an opportunity for a team member to join our team in a newly created role, titled Support Facilitator. Full time Role.Tasks, duties and responsibilities:
You will be reporting to the Manager, Support and Resources, you will be responsible for providing professional support, needs assessments and reassessments and arranging support packages for clients. You can identify and prioritise client support needs and coordinate the information and planning process for people with needs, ensuring they are engaged with the right services and support at the right time. Your goal is to ensure that the right outcomes are being achieved for clients and their family / whanau.About you:
You're a fantastic communicator with a collaborative nature, which goes hand in hand with your proven assessment experience in disability or mental health. You have a strong understanding of the AOD, general health or community service sector and your ability to easily build rapport with range of stakeholders and clients, will set you apart from your peers.It would be ideal if you have a specialised area of interest and expertise such as dual mental health and disability, aged / palliative, physical and / or sensory, or high and complex needs.
It's critical that you have well-developed communication and interpersonal skills and excellent time management and organisation skills. And of course it goes without saying that you are self-motivated and passionate about what you do!
