The Hawkes Bay Older Persons Health Directorate:
The talented team who work in our 30-bed Assessment, Treatment & Rehabilitation ward have recently transformed the model of care, and now deliver even better, person-centred, goal-based rehabilitation. The inter-professional team consists of nurses, allied health professionals and assistants, 3 Geriatricians, one Registrar and two house officers.
Our award-winning engAGE Community Service for Older People has 6 locality-based multi-disciplinary teams where we work with our primary care colleagues, home care providers and Needs Assessment Services to maintain the independence of older people in our community. Our teams work in people's homes as well as in our Intermediate Care Beds. The ORBIT, inter-professional allied health team, target older people presenting to the Emergency Department and Acute Assessment Unit for rapid assessment, 7am-7pm, 7 days per week, and act as a key link between hospital and the community. Our 2 community Geriatricians and 3 Gerontology CNS's work with these teams and also provide care through clinics, home visiting and residential care visiting services. We work closely with our colleagues in the Department of Medicine and with the Older Person's Mental Health team.
We also have big plans for the future! The Hawkes Bay health sector Clinical Service Plan has highlighted older people as a priority population, and our vision is a whole-of-patient journey approach to delivering care. Working with the team to develop and implement innovative new services, including Early Supported Discharge, will be a key part of this role.
Hawkes Bay is accredited by the RACP for Advanced Training of Registrars in Geriatric Medicine, and we plan to have House Officer community based attachment in Older Person's Health commencing 2019/20. On-going development is important to us so an active role in student and junior doctor teaching programmes and education and CME activities within Older Persons Health and the Department of Medicine will be an important and rewarding part of this role.

The successful applicant should have a FRACP with specialist training in geriatric medicine (or equivalent) and be registered (or registerable) as a specialist with the Medical Council of New Zealand.
