Discounted Health Insurance Online learning platform You will be working as part of our CREST Nursing service as a Case Manager to provide personalised community-based assessments to newly discharged clients with a restorative approach. This involves identifying and resolving issues as and when they arise to maximise rehabilitation potential. This short term service aims to return clients back to their baseline level of function and if that is not possible, to plan for long term support. This role is about working with our already established team of Nurses, Coordinators, Support Workers and other Allied Health within the community setting. As a Registered Nurse you will provide a broad range of care to clients in their own home such as: Passion to deliver exceptional services to people with health or injury related needs in their own homes.
