Enjoy the autonomy of managing your daily work schedule.About the Role:As a Home Care Physical Therapist, you will:Provide both direct and indirect patient care to facilitate the rehabilitation and restoration of normal body functions.Implement personalized treatment programs based on each patient's plan of care.Administer therapeutic modalities and train patients in therapeutic exercises, transfers, and gait.Supervise and guide PT Assistants and PT Aides to ensure the highest quality of care.ResponsibilitiesPhysical Therapy Assessment and Skilled Treatments.Client Care Coordination.Teaching, Supervision and Counseling Clients and Caregivers.Documentation of Interdisciplinary Communication and Supervision of Ancillary staff.Problem Solving and Critical Thinking Skills.Ability to effectively and safely transfer patients with significant physical and/or mental impairment.Time Management and Development of Plan of Care.QualificationsAbility to assist in patient lifts, transfers, and guarded functional activity training.Minimum required: Bachelor's degree.Preferred/desired: Master's degree or doctorate.Competencies and SkillsClear Communication Skills (Both Written And Verbal).Able To Keep Confidential Information Regarding Patients and Team Members.Able To Withstand Crisis Situations.Skills to Provide Customer Service To Patients, Team Members and Visitors.Proof Of Current/Valid Car Insurance.Knowledge And Experience With Electronic Health Records.Experience With Excel, Power Point, Word, Visio, Etc.Valid Driver's License With No More Than 3 Moving Violations.CredentialsEssential: PT - Physical Therapist, BLS - Basic Life Saving certification.EducationEssential: Bachelor of Science (BS).Other InformationEntry: USD $38.81/Hr.Expert: USD $60.16/Hr.
