Job Description - Pacific Nurse Case Manager (HAW07850) Pacific Nurse Case Manager Vacancy No Vacancy No : HAW07850 Success is not the work of one, but the work of many. 12 Months Fixed-Term 80 hours per fortnight (1.0 FTE) A Pacific Nurse Case Manager position has become available in our team. This role sits within the Pacific Health clinical team made up of Pacific Health Navigators and Registered nurses based at Hawke's Bay Fallen Soldiers' Memorial hospital. Using Pacific Health Models of care, the team work to deliver person and family centred care to support Pacific Peoples to confidently navigate the health system and affect good outcomes. This in turn helps Pacific health consumers to take control of their own health and wellbeing and works in partnership with local agencies, services and providers. You will also have opportunities to support and input into community led initiatives as part of the Pacific community health and wellbeing village and to share with other primary and secondary care clinical teams, services and our public health team. If you are a Registered Nurse and have an affinity for and a working knowledge of Pacific Peoples, worldviews, languages, customs, and cultures along with excellent communication and relationship building skills, then we encourage you to apply. What you will bring You will be confident working across diverse environments and clinical teams and have good interpersonal and communication skills. You will also bring: A current New Zealand nursing qualification Have 3 years+ of relevant experience as a Registered Nurse Eligibility to work in New Zealand is mandatory Commitment and understanding of Pacific cultural values, practices and principles Basic proficiency in one or more Pacific language's Have a good command of written and spoken English What's in it for you?
