Nurse – Long Term Conditions: Diabetes, Respiratory, Cardiac

Located Horowhenua Health Centre, Levin

Due to someone taking leave without pay, we have a fixed term position available for an experienced Nurse specialising in Long Term Conditions (LTC) to be located at Horowhenua Health Centre, Levin. The working hours are 20 to 36 hours per week - we are flexible, however require the successful applicant to hit the ground running so proven experience across LTC is essential.
This varied role will see you assessing, supporting, and managing LTCs to clients / family and whanau throughout the community.
The FTC will suit an experienced Registered LTC's Nurse and as such we are keen to speak with applicants that possess the following skills:
An experienced Registered Nurse with a current annual practising certificate
Proven experience in LTC - Diabetes, Respiratory, Cardiac and keen to practise across this cross-section of conditions throughout the Horowhenua community
Experience running educational programmes - this is desirable
Practical approach
Professional manner & ability to build trust, respect and rapport with patients
A genuine interest in the Primary Health Care sector
