Northland District Health Board advocates for affirmative action and actively seeks Maori into our workforce to reflect the community we serve.
The Nursing and Midwifery Directorate are seeking a senior registered nurse with a current APC to join the team to provide local/regional coordination, development and facilitation for advance care planning (ACP) with a focus on cross sector engagement in Northland.
Advance care planning is the process of thinking about, talking about and planning for future health care and end of life care.
The key functions of this role are to build confidence in ACP use across primary care, secondary care and community settings and to ensure that advance care plans are made available to health providers on the patient information system.
We are seeking someone with experience in education coordination, data analysis and report creation. You will need to be very organised with high-level computer skills (Microsoft Office, Word and Excel) as well as possessing excellent verbal and written communication skills.
