About the service
Disability Support Link is the Needs Assessment and Service Coordination agency for Waikato District Health Board (DHB). We are comprised of two teams; the Health of Older People team and the Disability Support Services team. We work closely with other older persons' services both in the hospital and the community.
About the role
We are looking for a registered health professional to work in our Health of Older People team supporting Kaumatua/Kuia/Whaanau in the Waikato DHB rohe (area). This will be a varied and exciting role working alongside Whaanau and Maaori providers in Hauraki/Thames/Coromandel, Maniapoto, Raukawa, Tainui and Pasifika based in Hamilton and Tokoroa. The role will include supporting Whaanau in the Waikato DHB hospitals and in the communities to ensure the cultural needs of Whaanau are met in the process of a needs assessment and service coordination.
The Health of Older People team covers a range of contracted support services for Kaumatua/Kuia with health and age-related disability needs, and younger Whaanau whose needs relate to chronic health conditions.
Prior experience working with InterRAI is an advantage but not essential as training will be provided.

Requirements
We are seeking a Registered Health Professional who has:
A current New Zealand Practicing Certificate
Proven ability to work with Whaanau from a cultural perspective
Passion, motivation and initiative to work alongside Whaanau, Hapu, Iwi
Tikanga values and Te Reo
Understanding of the Tiriti O Waitangi
Worked as a roopu to improve and break down the barriers for Whaanau accessing services in the community
Networking and maintained relationships with Maaori and Pasifika providers
Proven assessment skills including comprehensive geriatric assessment
Excellent written and oral communication skills
Previous experience working in disability or gerontology sector
A current full New Zealand drivers licence
