**About the Role**As Operations Manager for Community Services, you will be the operational leader for District Nursing, Community Assessment & Rehabilitation Service (CART), Supported Discharge Service (SDS), and Specialist Nursing services.
You will provide leadership and oversight of community-based health services, ensuring seamless, person-centred care, effective discharge planning, and continuity of care for complex and vulnerable populations.You will work closely with the Clinical Managers across the services, as well as community partners.
