Integrated Services Case Manager

We're looking for a candidate to this position in an exciting company.

Meeting clients in prison, Community Probation, or Work and Income service centres
Helping clients identify their goals and make plans to achieve them
Assisting with MSD products and services
Making referrals to and attending hui with providers in health, housing, and whanau support
Connecting clients to suitable employment, education, and training.

Have experience in case management (social sector or otherwise)
Have a passion for stronger whanau and communities
Have a clear understanding of multicultural New Zealand to ensure collaboration
Have established networks and a proven record in a similar role
Demonstrate excellent written and verbal communication skills
Have a full New Zealand driver licence without conditions.
