Integrated Services Case Manager

We're looking for a candidate to fill this position in an exciting company.

Working closely with whanau and individuals to create tailored plans, set goals, and staircase towards securing long term housing
Maintaining high levels of client engagement to encourage and support clients to achieve their goals
Increasing opportunities for clients through understanding of housing support available across all agencies, including MSD and Govt
Assisting clients to identify and address health and social needs, linking them to appropriate services such as budgeting, mental health, addiction, family harm support.
Working collaboratively using a multi-agency approach, partnering to support the client's goals.

Sound understanding and awareness of who is in the housing sector
Demonstrated history of working with other organisations to achieve successful outcomes
Excellence in networking and building effective partnerships, both internally and externally
Strength in building trust and rapport with at risk whanau and individuals
Strong cultural awareness, upholding the values of Manaakitanga
Exceptional client engagement skills with demonstrated abilities in planning and staircasing
Ability to work constructively to influence positive outcomes
Sound judgement and decision-making abilities
Strong housing focus with great understanding of MSD products and services
