We have an exciting opportunity for a registered or enrolled nurse to join our Manukau locality interdisciplinary team based at Home Health Care Papakura. We are seeking qualified, experienced and flexible individuals.
This role encompasses a wide range of skills such as advanced wound care management, continence management, rheumatic fever prophylaxis and palliative care. We have a reablement ethos which is a strengths based approach which supports patients to be well in the community by focusing on functional improvement and participation. We also now have Hospital in the home which gives opportunities to use your acute nursing skills in the community. We are looking for an adaptable person with the ability to work autonomously using evidence based practice.
You will be based at Papakura and occasionally you may be required to work across other Community Health Team bases.
You will have the support of the interdisciplinary team and access to ongoing in-service education, professional development and supervision provided by senior colleagues.
This is an exciting time to be working in primary care in integrated community health teams. If you have the enthusiasm, we can provide the support. You will be given a comprehensive orientation.
Essential
Current Annual Practicing Certificate
Current Full NZ Drivers Licence
A strong commitment and genuine interest in quality and service
The ability to work independently and be a team player

Desired
NZ Registered Nurse with 2+ years' experience
Post Graduate qualification or equivalent in relevant clinical area
Proficient level or higher on the CCP (RN pathway)
