This is a newly created job working with a recently established and significantly resourced integrated care team which aims to strengthen the management of diabetic patients in the community co-ordinating medical care with the primary care physician. The team has two main functions: Clinical Expertise in the management of complex diabetic patients. Support, education and training for both health professionals and patients, guided by the needs of Primary Care and Maori providers. Role Responsibilities provide clinical expertise and support to the Diabetes Integrated Team (DIT) to deliver care to patients in accordance with the Ministry of Health Quality Standards for Diabetes Care. work collaboratively across partner organisations from multiple locations including the hospital environment, community and within general practice clinics. carry a caseload of patients and provide targeted support, guidance, education and advice to the health workforce as the professional diabetes clinical expert. promote and role model excellence in diabetes clinical leadership and professional management including elevating the standard of diabetes care across the Diabetes Integrated Team and sector. Manage Type 1 and 2 and gestational diabetes in pregnancy working with a multidisciplinary team including obstetricians, midwifes as well as the diabetes nurse specialists. Prioritise preconception management where possible and support for the maternity team throughout the pregnancy and then post-natal management and advice following relevant guidelines Requirements: To be successful in your application for this opportunity as the Consultant Endocrinologist we are looking to identify the following on your profile and past history: QUALIFICATIONS: medical degree and FRACP or equivalent higher qualification in relevant endocrinology PROFESSIONAL REGISTRATION: must be eligible for vocational registration with the medical council of New Zealand. SKILLS: must be able to work effectively as part of a team but also independently and have enthusiasm for the teaching of junior doctors.
