About the RoleWe are seeking an experienced community support worker to work in a new community based team- Te Whakawhitinga.
Te Whakawhitinga is a co-ordinated primary social service aligned to the Te Whatu Ora Waiora vision and underpinned by Kotahitanga.
The Kotahitanga values of unity, togetherness, shared vision, working together, trust, relationships, collaboration and integrations are fundamental to the service model of Te Whakawhitinga.
We intend to meet the needs of tangata hapori/ tangata whaiora across the housing continuum, who are experiencing health, social and environmental issues that could put tenancies at risk or have resulted in homelessness.
We will do this by being a tangata hapori/ tangata whaiora led service that; helps a person identify their goals and aspirations that will achieve mauri orasupports access to community resources and services that will meet these goalsworks in close partnership with tangata hapori/tangata whaiora and other agencies in a transparent and coordinated way.
About you Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth.Experience in upholding Te Tiriti principals and frameworks, and the application of Matauranga Maori and Kaupapa Maori approaches, particularly as they apply in healthcare settings.An experienced community support worker.Ability to follow direction from registered staff to enthusiastically offer practical support for tangata hapori/ tangata whaiora to achieve good health and social outcomes.Excellent rapport and relationship building skills.Whanau centred, collaborative and partnership approach.Non-judgemental and empathetic.Competency with te ao Maori, tikanga, and te reo Maori or a commitment to starting your journey and taking ownership of your learning and growth.Experience working in a mental health, community health or primary care setting.Experience working with Maori and Pasifika groups.Highly developed interpersonal communication and negotiation skills including demonstrated ability to build productive working relationships with colleagues to achieve required outcomes.
