Community & Older Persons Social Worker at Te Whatu Ora - Health New Zealand Hutt Valley, Lower HuttNau mai, haere mai ki Te Whatu Ora - Capital, Coast & Hutt Valley District Contract: Permanent, Part Time (0.9FTE or 36 hours per week / 72 hours fortnightly)

Any non-Te Whatu Ora applications will be declined. This permanent position has become available following a re-allocation of hours within the team.
In this role, you will be working with clients/whanau as they recover or learn to live well with a wide range of health issues.
Using a strengths-based and client-centered approach, you will support our clients and communities as they navigate the many social stressors that impact on well-being.
You will also use brief interventions to help people build the emotional and psychological resilience they need and link them to the practical resources they require.
Te Herenga Tangata is the Community Older Adults, Rehab and Allied Health team for the Hutt Valley.
We are committed to building relationships and working constructively with a wide range of Community partners in order to meet the needs of our people.
Beyond Social Work specific practice, the team is continuing to develop inter-professional practice approaches so (following the relevant training), you will also have the opportunity to engage in tasks that will relieve other professionals within the team of unnecessary interventions.
