Permanent 80 hours per fortnight

The mental health and addictions service deliver a variety of services across the Waikato region. We are a service undertaking a huge programme of change to deliver better outcomes to tangata whai ora and whanau in our region. Nga iwi Ngakaunui - people at the heart is demonstrated best in mental health and addictions, people really are at the heart of all that we do. We know how important strong relationships are; with people who access our services, with their whanau and with the communities we serve.
The Integrated Care Coordination Team (ICCT) team supports the transition of long term community mental health service users from secondary mental health services into primary care settings.
The role has a strong emphasis on consultliaison, relationship development, brief interventions and wrap around support. We are also looking to develop the role and continue to seek the best way to support our Tangata Whaiora.
Ideally you will be an experienced and dynamic mental health professional that is familiar with coordinating care and providing specialist advice to G.P’s and practice nurses around the mental and physical health needs of Tangata Whaiora. You will be confident providing education and liaising with a wide range of people and services and negotiating shared care arrangements. You will have a strong commitment to promoting positive relationships, quality improvement and collaboration between all key stakeholders.
Applicants must have registration with the appropriate registration body and hold a current full, clean driver’s license.
