Te Uru Whakamauora Directorate have an opportunity for a senior nurse to join the Older Adult Mental Health Community team at an exciting time of development for community-focused, culturally responsive, integrated health care for kaumatua and older adults in our region.
This role includes working in the community with adults over 65 years and those of Maori and Pasifika descent over 55 years.
We are seeking to recruit a senior nurse who can ensure they deliver clinical and cultural best practice across the region.
This role will be based in Palmerston North but supported by a team of nurses who engage in comprehensive assessments and support, within the care continuum in a variety of care settings including people's homes, throughout Te Pae Hauora o Ruahine o Tararua, MidCentral.
Responsibilities: Deliver clinical and cultural best practices.
Engage in comprehensive assessments and support.
Work collaboratively with multidisciplinary and transdisciplinary teams, community organisations, and cultural groups.
Minimum Requirements: A minimum of 5 years of recent clinical experience in nursing roles that include working with people with dementia and functional mental illness, and frailty in a secondary care setting.
Advanced nursing expertise, clinical assessment, and coordination of support.
A relevant Masters qualification with prescribing qualifications (or working towards in an agreed timeframe) that includes postgraduate courses in the specialty of mental health nursing.
Current driver's license with no restrictions.
