The role: Achieving the best health outcomes for our patients starts with expecting the best from ourselves, and right now Te Tai Tokerau / Northland is looking for a motivated Senior Nurse specialising in wound care to join our District Nursing Service.
The Nurse Specialist is an advanced nursing practice role and is responsible for providing specialist nursing care and/or coordination of care to a specific patient/client population across healthcare settings.
This position will work in the community to support patients needing specialist wound care, and management of conditions affecting viability and integrity of skin and underlying tissues.
The CNS will work in partnership with referred patients and their whanau, as part of a multidisciplinary team across the services in the Directorate along with other specialist and Primary care teams Case management and coordination of the patient and their whanau with health professionals, inpatient areas and intersectorially is integral.
This role will be an expert in assessment skills; diagnostic reasoning, knowledge of management options and monitoring that underpins practice.
They will have specialist knowledge in complex wound care and lower limb management, and health conditions central which are central to the role.
This is a permanent, part time position (0.9 FTE) working 36 hours per week based in Whangarei, however some travel around the region may be required.
