Royal District Nursing Service New Zealand (RDNS NZ) offers an innovative, progressive and professional environment.
RDNS NZ is a leading provider of home based restorative models of care across the Auckland, Southland and Otago areas that work alongside our parent company in Australia.
We are looking to appoint a Clinical Coordinator to our Southland team. This position can be based in either Gore or Invercargill.
A significant part of this role involves conducting comprehensive assessments, identifying the support and rehabilitation needs of referred clients and options to address these through the development of individualised service plans. This role requires you to work closely with an experienced team of support workers.
Ideally your skill set will include:
Registered Health Professional with Current Annual Practicing Certificate
Experience in Community Health Services
Gerontology knowledge and experience
Full driver's licence
Intermediate level Microsoft Office skills
Strong communication skills
Being highly organised and able to manage competing demands
Recent experience in Clinical Coordination and InterRAI suite of tools would be advantageous
