Tui Ora is a holistic kaupapa Maori primary care organisation, providing a range of whanau centric health and social wellbeing services within our Taranaki community. Our Tui Ora Hauora a-Whanau (Family Health Medical Centre) serves a broad range of approximately 5000 registered patients. Our patients feel the difference when engaging Tui Ora services. Mo te turanga | About the role The primary purpose of this role is to facilitate high quality coordination of care for whanau with the greatest complexity of care needs. These whanau require their care to be frequently transferred between providers and across services, which includes hospitalisations. Facilitating care coordination improves quality of care and can increase appropriate community-based care, closer to home. Effective internal and external care coordination can reduce unnecessary hospitalisations, re-hospitalisations, missed outpatient appointments, and perpetuation of the health inequities our whanau contend with. Kia ekena a angitu | To promote success in this role, you will have The ability to demonstrate practical understanding of the aspirations and challenges facing Maori communities Experience in applying Tikanga Maori as it applies in the role is desirable Well-developed written and oral communication skills Computer literacy in Microsoft packages including Word and Excel A working knowledge of a Client Management System (preferred) Excellent interpersonal skills and able to work as part of a team The ability to engage effectively with a number of external allied services Excellent organisation skills and able to take responsibility for completion of tasks A strong work ethic, demonstrated by a high level of integrity, reliability, efficiency, and self-management Excellent attention to detail Able to handle sensitive and confidential information.
