Summary of program and its objectives:
The Brotherhood aims to be the leader in aged care in Victoria, not only in the provision of quality services but also in undertaking research, policy development and advocating on behalf of our consumers and the sector. At the Brotherhood the focus is enabling older people who access our services to flourish. Our service model is based on person centred care and the Enhancing Capabilities Approach which ensures that older people experience a trusting environment that provides opportunities and choices that enable them to live the lives they value and maximise the older person’s independence and autonomy. This encompasses valuing diversity including cultural preferences, sexual orientation and gender identity.
Residential Aged Care provides accommodation and other types of support including healthcare support, dementia management and lifestyle and enrichment programs. These include:
Sambell Lodge - 43 bed facility located in Clifton Hill (redevelopment to 115 bed)
Sumner House – 43 bed facility located in Fitzroy
Main responsibilities of the role:
The Clinical Care Coordinator ensures the effective and efficient delivery of care services to residents in accordance with professional standards and legislative requirements, and with the support of Registered Nurses, Enrolled Nurses and Care team. As part of the Clinical care team. this position provides leadership in the development, implementation and evaluation of resident’s lifestyle and clinical policies and procedures, clinical governance, staffing models, skills mix and relevant employee competencies. This position plays a key role in ensuring the Brotherhood’s residential facilities achieve and excel against the Aged Care Standards. This position may be required to work across residential facilities on an ad- hoc basis or as a backfill.
To meet the selection criteria, candidates will require:
Essential
Registered Nurse with current APHRA registration and significant experience in residential aged care
Relevant experience and knowledge of all aspects of gerontological clinical care as well as understanding and management of aged care quality standards, ACFI and accreditation processes
Sound people management skills with the ability to lead multi-site and multi-functional team
Well-developed interpersonal and communication skills with the ability to build effective relationships and liaise across all levels both internally, externally and with people from diverse backgrounds
Proven ability to identify and resolve problems and make appropriate recommendations
Excellent organisational and time management skills with the ability to plan workload, prioritise and meet deadlines
Demonstrated knowledge and competency in Microsoft Office Programs including Word, Excel and Outlook
Understanding of and empathy with the values and ideals of the Brotherhood.
Desirable
Post graduate qualifications in gerontology, clinical management, health management
This is a full time position working 38 hours per week.
Salary will be in the range of $81,329.04 to $85,306.00 per annum, based on skills and experience plus superannuation. Attractive salary packaging provisions are available to increase take home pay.
