We have an exciting opportunity to appoint a go-getter to join our team in the Tauranga branch. Here at HealthCare of New Zealand Ltd we have been around awhile, coming up 30 years in fact, and are the largest Home Based Support provider in New Zealand. We need an individual who has an aversion to shift work and wants to be part of a relaxed team who deliver the best possible home-based supports to the community. Is that you?
About the position
This role is about working with our already established (and brilliant) team of Coordinators, Support workers and Clinical Advisors. As a Registered Health Professional you will ensure that our clients receive person centred support by creating and reviewing goal based service plans, providing staff training and clinical oversight and ongoing review and management of clients. Don't worry, we have a Responsive Care Model in place, it's called 'Good to be home' and it's pretty effective!
This is a role where you will have a wide variety: visiting clients and making sure they are receiving the exceptional support, liaising with other community providers, problem solving and connecting with the wider HealthCare of NZ team.
Sounds like a blast! If you have had experience in working with goal-based models within the community - that's a plus! If you are passionate about the community and its people - even better!
Down to business, the successful applicant will demonstrate the following:
Clinical assessment and decision making skills.
Exemplary professional judgement.
Commitment to working in an interdisciplinary team.
Confidence in leading and guiding other health team members.
Minimum of three years post registration experience.
Proven ability to engage and relate to Maori clients and service providers.
