If you are a registered health professional experienced working in the community who has a strong focus on empowering people to improve their health and well-being, maintain their independence and achieve their goals, we invite your application for a position on our team.
The role focuses on working with clients (and their families / whanau) living with chronic health conditions who have a wide range of high and complex needs; you will be working with clients ranging in age from 18 to 64 years of age.
Facilitating holistic assessments to identify client’s needs and mitigate risks and then co-ordinating services are key responsibilities of this role. The assessment tool used is the online interRAI Home Care tool which requires proficiency in Information Systems.
The Case Manager role includes establishing and developing networks with a wide range of individuals and organizations involved in supporting the client and their family / whanau, with regular reviews to ascertain the effectiveness of services provided.
The ability to ‘think outside the box’ and develop creative solutions to meet complex needs is essential.
Three case managers are based in Tauranga and one in Whakatane so depending on workload and leave requirements Tauranga team members do travel to Whakatane at times.
