This is an opportunity for a Care Coordinator with experience in healthcare management to join a high-performing team. Qualifications & experience To be successful in this role, you will have: A background in nursing, social work, or any other allied health professional. About You Exceptional communication and attention to detail Willingness to take on new tasks to support your team Ability to work both on your own and as an integral team member Ability to work under pressure Warm and friendly with a flexible approach Honest and reliable Tasks & responsibilities Get to know the patients and their families and make them feel welcome Coordinate the care of patients identified at greatest risk of poor health outcomes, or hospitalisation, using an interdisciplinary approach Monitor and coordinate treatment plans with the patient and their care team Educate patients about their illness and connect them with medical professionals as and when needed Assist the care team with setting goals for quality assurances and best practices Work as a team player to ensure each patient receives the best care possible  Apply Now!
