The Cancer Nurse Coordinator will be responsible for the coordination and facilitation of individual patient pathways across the primary, secondary and tertiary care interface, establishing effective working relationships with regional members of the multi-disciplinary team, patients, their families and care givers. To be successful in this role you will be:
Able to use advanced communication, clinical knowledge and applied reasoning to ensure optimal patient care coordination during their cancer pathway.
Able to work actively, in partnership, with the medical, nursing and allied health staff across the care continuum to ensure safe, patient focussed care.
A self-directed team player who will be responsible for working closely with clinical and management staff from each cancer tumour stream in order to coordinate management of the process through the cancer patient pathway from high suspicion to first treatment date.
Currently working at least at a Proficient nurse level with 3 years recent nursing experience in this area.
Have completed or be undertaking Post Graduate study in a relevant area.
Possess a flexible can-do attitude, a passion for innovation, a high level of attention to detail, and a focus on providing quality patient focused outcomes.
Requirements:
Registered Nurse with the NZ Nursing Council
Valid annual practicing certificate
3+ years relevant nursing experience, post registration
Adult cancer nursing experience
