Acute care Co-ordinator role

We are seeking a dynamic, agile and thoughtful nurse or allied health clinician to develop and shape this newly created role.
Registration with the New Zealand professional body and a current Practicing Certificate are essential.
As the community acute care co-ordinator you work with a wide variety of teams and individuals, across a diverse range of service groups and non-government organisations (NGO) to co-ordinate and manage admissions to Te Whetu Tawera and acute alternatives, including respite during business hours.You will have excellent communication skills and a natural ability to develop strong and collaborative stakeholder relationships. Ensuring timely, equitable access to effective care and treatment for service users and their whanau will be your priority in providing whole of directorate co-ordination across all adult service groups.
